Pakistani MBBS to Doctor in Germany: The Approbation Pathway

Your MBBS can be the start of a medical career in Germany, but practising as a doctor requires German recognition, strong medical language skills, and a route that depends on your chosen federal state.

There is opportunity — and a proper process

Germany needs medical professionals, and many internationally trained doctors work here. But medicine is a regulated profession: a Pakistani MBBS is not an automatic licence to treat patients after arrival.

You apply for recognition through the responsible authority in the federal state where you plan to work. That authority assesses your qualification and tells you the exact documents and any compensatory measures required.

Your route from MBBS to Approbation

Some steps overlap. For example, you can prepare documents while learning German. Treat this as a planning map, then confirm your exact route with your state's recognition authority.

    Choose a state

    Pick the Bundesland where you want to work and find its Approbation authority.

    Prepare and apply

    Submit the Approbation application and the documents requested by that authority.

    Build medical German

    Reach B2 general German and prepare for the medical Fachsprachprüfung (FSP).

    Receive the assessment

    The authority checks whether your education is equivalent to German medical training.

    Address any gaps

    If required, pass the Kenntnisprüfung (KP). A temporary Berufserlaubnis may be possible in some cases.

    Receive Approbation

    Once all requirements are met, you receive the unrestricted licence to practise.

The three major milestones

1. Recognition assessment

The authority compares your qualification, experience, and documents with the German reference qualification. Send only the documents it requests, in the required form and translation.

2. Fachsprachprüfung (FSP)

This assesses medical communication in German. The usual standard is C1 medical German, alongside the B2 general German requirement.

3. Kenntnisprüfung (KP)

If the authority identifies substantial differences, it may require this medical knowledge exam before it can grant Approbation.

Visa and budget

Your visa route depends on your situation: for example, a signed job offer, a recognition measure, or another valid residence purpose. Do not choose a visa type from social-media advice alone; check the current requirements with the German mission handling your application.

Build a personal budget that includes document translations and certifications, recognition and exam fees, German courses, visa costs, travel, health insurance, and living costs. The total can vary widely by state, city, language path, and how many exams you need.
